By opting in to receive communications from any of the Companies, you agree to the following Terms and Conditions:


1. CONSENT TO RECEIVE COMMUNICATIONS (A2P)

By providing your phone number and opting in, you:

Expressly consent to receive SMS/MMS messages, phone calls, and notifications from the Companies

Acknowledge that messages may include: Appointment confirmations and reminders Treatment-related information Administrative updates Promotional messages (where applicable and permitted)

Consent is not a condition of receiving medical or dental services.

Ongoing services or promotions


3. MESSAGE AND DATA RATES

Message and data rates may apply depending on your mobile carrier

The Companies are not responsible for any such charges


4. OPT-OUT (CANCELLATION)

You may opt out at any time by:

Replying STOP to any message received

After opting out, you will receive a final confirmation message and no further messages will be sent unless you re-enroll.

However, standard SMS and MMS messaging may not be fully secure

By opting in, you acknowledge and accept that:

You assume the risk of receiving such communications on your mobile device

You are responsible for maintaining the privacy and security of your device

The Companies will make reasonable efforts to limit PHI exposure and use appropriate safeguards where possible.


8. LIMITATION OF LIABILITY

To the fullest extent permitted by law:

The Companies do not guarantee uninterrupted or error-free messaging services

The Companies are not liable for: Delayed or undelivered messages Errors caused by mobile carriers or third-party providers Unauthorized access to your messages or device

Communications provided are for informational purposes only and do not constitute medical or dental advice.


9. USER RESPONSIBILITIES

By using these services, you agree that:

You are the authorized user of the phone number provided

You will notify us promptly of any changes to your contact information

You will not misuse the communication services


10. MODIFICATIONS

The Companies reserve the right to update or modify these Terms at any time.
Updates will become effective upon posting.


11. GOVERNING LAW

These Terms shall be governed by the laws of the State of Florida, United States.
